1.需主动提供自身健康状况,过去病史,家族史,过敏史等让医护人员了解.You are required to voluntarily provide the personal information about your health conditions, past medical history, family medical history, and allergic history to the medical professionals.
2.对於经您同意的治疗,应配合医护人员的医嘱.
For the treatment which is under your permission, you are supposed to comply with the advice from the medical professionals.
3.希望您能尊重专业,不要求医护人员提供不实的资料或证明.

After taking the train to Gangshan station, you can take a taxi to the hospital.

After taking Kaohsiung bus to Gangshan bus station, you can reach the hospital by taxi.
自行开车,於中山高速公路冈山交流道下,沿县186号道(介寿东路)经介寿路,右转进大义二路即可到达.
If driving, you can exit the Freeway 1 at Gangshan interchange. Driving along the County Rd. #186 (Jieshou E. Rd.), passing through the Jieshou Rd. and turning right at the Dayi 2nd Rd., you will find the hospital on the right.

96年全民健康保险保险对象应自行负担之住院费用上限如下:依据中央健康保险局95年11月28日健保医字第0952600491号函,96年全民健康保险保险对象应自行负担之住院费用上限,业经行政院卫生署公告(请见如下说明),订自96年1月1日起实施:
依据:
一, 全民健康保险法第35条.
二, 全民健康保险施行细则第64条及第70条之1.
公告事项:
一, 96年全民健康保险保险对象因自行负担之住院费用上限如下:
(一) 因同一疾病每次住院部分负担上限:2万6,000元.
(二) 每年住院部分负担上限:4万4,000元.
前列住院部分负担上限之适用范围,以保险对象於急性病房住院30日以下或於慢性病住院180日以下所应自行负担之医疗费用为限,不包含全民健康保险法所规定不予给付之项目.
二,本规定自96年1月1日起实施.
慢性病连续处方笺给药规定
慢性病连续处方笺之患者,请於上次给药用完前7日或后7日内(含例假日),持原处方笺至门诊批挂柜台取号办理.依全民健康保险医疗办法第四十三条之规定:保险对象持慢性病连续处方笺调剂者,须俟上次给药期间届满前七日内,始得凭原处方笺再次调剂.前项保险对象如预定出国超过一个月,得於领药时出具机票等证明文件,先行领取下个月之用药量,但当次之全部给药量以二个月为限.

1.需主动提供自身健康状况,过去病史,家族史,过敏史等让医护人员了解.You are required to voluntarily provide the personal information about your health conditions, past medical history, family medical history, and allergic history to the medical professionals.
2.对於经您同意的治疗,应配合医护人员的医嘱.
For the treatment which is under your permission, you are supposed to comply with the advice from the medical professionals.
3.希望您能尊重专业,不要求医护人员提供不实的资料或证明.

After taking the train to Gangshan station, you can take a taxi to the hospital.

After taking Kaohsiung bus to Gangshan bus station, you can reach the hospital by taxi.
自行开车,於中山高速公路冈山交流道下,沿县186号道(介寿东路)经介寿路,右转进大义二路即可到达.
If driving, you can exit the Freeway 1 at Gangshan interchange. Driving along the County Rd. #186 (Jieshou E. Rd.), passing through the Jieshou Rd. and turning right at the Dayi 2nd Rd., you will find the hospital on the right.

96年全民健康保险保险对象应自行负担之住院费用上限如下:依据中央健康保险局95年11月28日健保医字第0952600491号函,96年全民健康保险保险对象应自行负担之住院费用上限,业经行政院卫生署公告(请见如下说明),订自96年1月1日起实施:
依据:
一, 全民健康保险法第35条.
二, 全民健康保险施行细则第64条及第70条之1.
公告事项:
一, 96年全民健康保险保险对象因自行负担之住院费用上限如下:
(一) 因同一疾病每次住院部分负担上限:2万6,000元.
(二) 每年住院部分负担上限:4万4,000元.
前列住院部分负担上限之适用范围,以保险对象於急性病房住院30日以下或於慢性病住院180日以下所应自行负担之医疗费用为限,不包含全民健康保险法所规定不予给付之项目.
二,本规定自96年1月1日起实施.
慢性病连续处方笺给药规定
慢性病连续处方笺之患者,请於上次给药用完前7日或后7日内(含例假日),持原处方笺至门诊批挂柜台取号办理.依全民健康保险医疗办法第四十三条之规定:保险对象持慢性病连续处方笺调剂者,须俟上次给药期间届满前七日内,始得凭原处方笺再次调剂.前项保险对象如预定出国超过一个月,得於领药时出具机票等证明文件,先行领取下个月之用药量,但当次之全部给药量以二个月为限.
